Bethesda surprise-released Fallout 4: FEVer Dreams on Oct. 20, 2026, making the free adventure available on PlayStation 5, Xbox and PC through Bethesda’s Creation platform. Polygon framed the release as a reason to return to the post-apocalyptic RPG after hundreds of hours.

Kinggath Creations developed the mod in partnership with Bethesda under the Verified Creations label. Polygon said Kinggath Creations made it in partnership with Bethesda, and the adventure’s description reads Presented by Bethesda Game Studios. The team released a Skyrim DLC earlier in the month, continuing a recent partnership pattern with Bethesda. Fallout 4 has been available for more than a decade.

Players choose one of two protagonists while the other spouse talks throughout, and new characters present heavy choices, including a ghoul’s dialogue about a single remaining FEV cure dose. Polygon said the trailer shows the non-chosen spouse talking to the player throughout, and IGN India quoted the official blurb mentioning conversations with their spouse. The adventure opens with the character trapped and unarmed while apparently suffering from an active strain of the Forced Evolutionary Virus. Players start the quest by finding a Gutsy robot in Diamond City’s market area after downloading.

FEVer Dreams takes place in an escape room-style dungeon filled with puzzles and promises lore about Fallout’s notorious bioweapon. Polygon said the escape room-style dungeon includes shooting elements alongside puzzles, and Push Square called it a psychological horror-tinged dungeon crawl. The facility is full of Deathclaws and mini nukes where perception plays tricks. It includes totally new gear and weapons alongside familiar faces, and the official trailer portrays the experience as a trippy odyssey that plays with perception.

Courtenay Taylor returns as the voice of the female Sole Survivor, and Brian T. Delaney returns as the voice of the male Sole Survivor. IGN Middle East specified Taylor voiced the female Sole Survivor and Delaney the male Sole Survivor. Push Square noted their return happens over a decade after Fallout 4’s launch, calling the actors’ return one last adventure.

Push Square emphasized the adventure costs nothing to download and install on PS5. The mods are add-ons to the base game, so Middle East players need their own copy of Fallout 4 on a platform that supports them. IGN Middle East noted FEVer Dreams itself is free with no regional price to check.

The release is part of Bethesda’s Fallout Day celebrations, with Fallout Day on Friday, Oct. 23. IGN Middle East said Fallout Day falls on Friday, October 23 and explained that date marks when bombs fell in 2077 in the Fallout timeline.

Fathomless from Zone Zero Creations is a cosmic horror Atlantic Ocean expansion coming in 2027. IGN India said it will be available in 2027. Its official blurb describes the sole survivor descending into a mysterious deep-sea crater after following an ambitious pre-war expedition. IGN Middle East quoted that blurb and said the expansion has over five unique enemy NPCs, a new underwater world space and interiors, and a new storyline with voiced dialogue.

Verified Creations represents Bethesda’s official support for modders building on Fallout 4, which is now 11 years old. IGN Middle East frames the trio of mods under this official program and cites the base game’s age as background for continued studio and community involvement. The announcement provides no regional pricing for Command Override or Fathomless and no release times for the Gulf or Egypt.

Fallout Shelter will receive a new Baby Deathclaw companion and new seasons. IGN India’s Fallout Day Celebrations 2026 coverage includes updates to Fallout Shelter alongside the three new Fallout 4 mods.

IGN Middle East and IGN said Bethesda is working on Fallout 3 and New Vegas remasters, Fallout 5, and an Obsidian-developed Fallout spinoff. Work continues on Fallout 76 and Fallout Shelter, and Season 3 of the Fallout TV series is incoming. The outlets said Bethesda expects to make big announcements around the series’ 30th anniversary next year.
